Life After Inpatient Care

Patient leaving Burke at discharge, just outside the hospital lobby with a doctor

Your time at Burke Rehabilitation Hospital is meant to help you recover enough to return home—and many patients do. Your journey may continue with outpatient therapy, home care, or at a sub-acute rehabilitation facility.

Call Your Case Manager

If you have any questions when you return home, please reach out to your assigned social worker or email Juliette Kleinman at jkleinman@burke.org
