Medical Records

If you are a current or former patient, obtain your medical records  by filling out this form and sending it to Burke’s Health Information Management Services. You can submit the form in several ways:

• Forms may be dropped off at the lobby in the Main Hospital, Building #7
• Send a fax to (914) 597-2773
• Email the form to anmadouty@burke.org
• Mail the form to Medical Records, Burke Rehabilitation Hospital, 785 Mamaroneck Avenue, White Plains, NY 10605
• The form is also available on My Chart
