Patients may be candidates for outpatient cardiac rehabilitation services if they have been diagnosed with a heart attack in the past 12 months, angioplasty and/or stent placement, heart valve repair or replacement, bypass surgery, heart transplant or a diagnosis of stable angina within the past the past 6 months.
A physician referral for cardiac rehabilitation services that identifies your cardiac diagnosis and date of onset is required. A referral can be faxed to our convenient outpatient facility:

Once a physician signed referral is received, we will conduct a medical and financial screening to determine insurance eligibility and financial coverage. The Burke Rehabilitation Hospital participates in a variety of insurance plans.(link) For more information about insurance-related issues, please call (914) 597-2206. Upon completion of the insurance verification we will contact the patient to schedule an appointment.
