Criteria for Outpatient Rehabilitation

You may qualify for the outpatient program if you meet either of these criteria:

  • You had a heart attack in the past 12 months
  • You had any of these in the past six months:
    • Angioplasty or stent placement
    • Coronary artery bypass graft (CABG) surgery
    • Diagnosis of stable angina (chest pain)
    • Heart valve repair or replacement
    • Heart transplant

How to Enroll in Outpatient Rehab

Ask your doctor for a referral to outpatient cardiac rehabilitation. Your physician must include your diagnosis and date of onset and then fax the referral to (914) 597-2801 or (914) 798-4151. After we receive it, we’ll perform a medical screening and confirm your insurance coverage. Then, we’ll call you to schedule an appointment.

Where to Go for Outpatient Rehab

Cardiac rehabilitation takes place on Burke’s main campus, Billings Building 4, at 785 Mamaroneck Ave. in White Plains.

Insurance Coverage

Most insurance plans cover heart rehabilitation. Medicare usually pays for 36 visits and may cover up to 72 sessions with a physician’s supporting letter.

After Outpatient Rehabilitation

Before you graduate from cardiac rehab, you’ll receive a customized home exercise plan. We may also recommend a follow-up maintenance program at our Fitness Center. Certified fitness instructors or exercise physiologists will guide and supervise your activity there to help keep you safe. You’ll have access to weights and exercise equipment.
