Physician Follow Up

For patients who require ongoing treatment by a specially trained physician, Burke offers the option of being seen by a patient’s Burke treating physician after discharge. Patients should follow up with their primary care physician upon discharge, but a Burke specialist may be recommended to further rehabilitation goals.  Specialists in all of the fields of rehabilitation are represented in the outpatient clinic including Stroke, Brain Injury, Dementia, Spinal Cord Injury, Multiple Sclerosis, Parkinson’s disease, and COPD/Lung disease.  

Appointments can be made with your social worker before you are discharged from Burke or can be scheduled by calling (914) 597-2332 after discharge.
