Burke can be your partner in managing the care of your patients, whether they are currently in an acute care hospital or living in the community. Rely on our rehabilitation knowledge and skills to treat patients post stroke, pre- and post-surgery, for neurological conditions, brain and spinal cord injuries, sports injuries, trauma, and general debility. A collaborative approach can lead to improved patient outcomes and experience, especially for individuals with complex medical and rehabilitation needs.

Hospital Admission

Most patients admitted to Burke Rehabilitation Hospital are transferred from an acute care hospital. Our Care Transition Coordinator (CTC) and a rehabilitation physician review medical records in support of the admission. For assistance, please contact our Inpatient Screening Department at (914) 597-2519.

Outpatient Physical, Occupational or Speech/Language Therapy

For PT, OT, or SLP, provide your patient with a prescription for therapy along with the diagnosis. Patients can choose a Burke outpatient location most convenient for them by consulting the locations directory and then calling to make an appointment.

Cardiac Rehabilitation

Fax a referral, including diagnosis and date of onset to (914) 597-2801

Pulmonary Rehabilitation

Fax the results of a recent pulmonary function test or spirometry and a prescription for pulmonary therapy to (914) 597-2763.

Consult with a Physical Medicine & Rehabilitation Physician
