Discharge to Skilled Nursing Facilities

Some patients who are discharged from an inpatient program at Burke will require additional inpatient care. Although not as intensive, sub acute care at a skilled nursing facility may be required for patients with serious illnesses or injuries who require more time for rehabilitation or skilled nursing care.

In 2010, Burke discharged 345 patients to skilled nursing facilities for ongoing care at the sub acute level. This represented about 14% of all discharges for the year.

When Additional Care is Needed

One of Burke's primary goals for patients is that they be able to return to the home environment from which they came. However, some patients who are discharged from an inpatient program at Burke will require additional inpatient care.
