At Burke, our inpatient programs are designed to be intensive to ensure that each patient makes the maximum gains and recovery that are possible in the limited time of an acute hospitalization. Our goal is always to discharge patients to the home environment from which they have come. In 2013, 2,461 Burke Rehabilitation inpatients were discharged to home. This represented 80% of all discharges.
Some patients are able to return home with minimal care, while others will require sub acute care at a skilled nursing facility; many patients will continue rehabilitation in an outpatient setting and others will seek out support groups and community programs to help with the adjustment after hospitalization.
At Burke, your relationship with us can continue long after discharge. Whether you require additional sub acute care at one of the Burke network facilities, choose to come to Burke for Outpatient therapy at one of our clinics throughout the community, join a free Burke sponsored support group or even enroll in a fitness program through our fitness center, our continuum of care offers patients the opportunity to remain in the Burke family for as long as they choose.
