The admissions process includes an assessment of the medical, clinical and psychosocial needs of each patient to determine the appropriateness of acute physical rehabilitation. Not all patients will be suited for this intensive rehabilitation program. A patient’s functional level is an important variable in this process. Some patients may be too low level while others may be too high functioning. Therefore each patient is screened by a medical professional.
The initial assessment process may be completed by phone or on-site visit by a Burke Rehabilitation Hospital Nurse Screener – A Registered Nurse. Burke has Nurse Screeners that visit area hospitals and can begin the screening and admitting process while a patient is still in an acute care hospital. By doing so, patients can be transferred directly to Burke when medically stable and appropriate for in-patient acute rehabilitation.
When an admission has been approved by your insurance company and Burke, our Screening and Admitting departments will work with your referring facility to arrange transportation and to assist you in completing the appropriate paperwork for admission.
