How do I become a patient in the Outpatient Department?

Generally, patients may be candidates for outpatient services if they have experienced loss of function due to injury or illness. Adults with speech, language, or swallowing disorders may also be candidates for these services. If your primary care physician, specialist or even inpatient Burke physician has recommended outpatient therapy, we are happy to assist you.

In order to begin the process of becoming a Burke “Outpatient” a referral from a licensed physician is required. It should, state a diagnosis and type of therapy prescribed. This must be done before an appointment can be scheduled. For your convenience, the prescription can be faxed to any of our outpatient facilities:
